About
Ensuring that review staff and administrators have access to records needed to complete robust reviews. Encouraging positive partnerships across the system to deliver the outcomes of LeDeR with a priority focus of Learning into Action. Preparing the Annual LeDeR Report for BOB ICB. The role of the Local Area Contact (LAC) is independent of LeDeR reviewers in all cases. Performing the Quality and Assurance oversight role for LeDeR reviews in BOB ICB. Work closely with the Improvement Manager to ensure there are effective assurance processes of LeDeR within the ICS. Take the lead in establishing the local governance group/panel which will meet on a frequency determined by the number of reviews and meets the assurance requirements. The group will have final sign-off on reviews completed for the ICB and determine the actions to be taken as a result of reviews as well as developing and monitoring the ICB LeDeR local action plans. Work within the ICSs and wider stakeholders to review and refine membership of key groups of senior people from across local health providers, commissioners and social care partners all of whom should have the authority to affect change and sign-off on reviews completed for the ICBs and determine the actions to be taken as a result of reviews (action from learning must be embedded) as well as developing and monitoring the ICB LeDeR local action plans. Ensuring that there is a BAME representative active within the LeDeR programme. Act as the lead contact for the NHS England regional coordinator. Lead on the promotion of LeDeR at a local level across health and social care. Ensure that the appropriate data sharing agreements are in place within the ICB to enable timely access to notes and work within the ICB to facilitate access to health and care notes for reviewers, for example through Smart Cards. Lead on the assurance of the quality of initial reviews, assure justification of initial reviews and sign off initial reviews on behalf of the ICB. Lead on the coordination of focused reviews to go to local governance panels to agree actions. Agree with reviewers any actions coming out of initial reviews (these should be infrequent), including confirming ownership of those actions and ensuring the actions are added to the LeDeR action plan for the ICSs and are followed up over time. Add the actions from initial reviews to the LeDeR web platform (these should not be numerous or frequent). Have responsibility for ensuring the SMART actions are uploaded onto the web-based platform once agreed at panel/ governance meetings. Raise concerns and ensure that any issues are raised at each ICBs quality governance meetings. Lead on the analysis of the data coming from reviews and be responsible for the development and delivery of the 3-Year strategic plan and annual report on behalf of each ICB for the ICB. Escalate any concerns as necessary and establish good working relationships with statutory processes to ensure that information is shared, and the response is co-ordinated between agencies, e.g., safeguarding adult reviews, serious case reviews, mental health homicide reviews, child death reviews, domestic homicide reviews.
