Diabetes Care: manage patients with Type 2 diabetes, insulin initiation, and complex long-term condition reviews. Core Practice Nursing: Deliver essential treatment room services including immunisations, wound care, cytology, and routine health checks. Collaborative Teamwork: Work closely alongside GPs, Advanced Nurse Practitioners, HCAs, and administrative staff across both surgery sites. Patient Impact: Drive proactive, long-term health outcomes for the Dordon and Polesworth communities.
