Job description
- About
The postholder will support patients through their hospital care journey, providing the patient with a single named point of contact who will coordinate their care.
Work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients’ journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust.
The role oversees the full administrative management of each patient’s clinical pathway from referral to discharge, ensuring all processes run smoothly, within agreed timeframes, and in line with Trust policies.
Responsibilities include maintaining accurate patient records on Cerner, coordinating appointments and investigations, monitoring clinic workflow, resolving delays, communicating effectively with patients, families, carers and clinicians, and arranging transport or translation services when required.
The post holder manages referrals, ensures pre‑investigations are completed, provides navigation support across sites, processes confidential documentation, handles concerns or complaints, and cross‑covers colleagues to maintain consistent service.
They also ensure high data quality, maintain up‑to‑date patient information materials, notify relevant teams of patient deaths, and support RTT data transfer when patients move to other trusts. Ongoing duties include attending meetings, completing mandatory training, staying proficient with electronic systems, and participating in annual appraisal and professional development.
