There are many great reasons to take your nursing skills to the community, including: work-life balance friendly hours 1-2-1 patient focused holistic care increase skills and experience, treating a variety of conditions and ages autonomous working friendly, supportive team make a difference closer to where you live Working within care homes and patients homes, you will be supporting the delivery of a wide range of clinically acute and chronic interventions, including end of life care, wound care management and long-term condition management as part of the wider multi-disciplinary team. You will be required to hold a current driving licence to access the pool cars, as well as have access to appropriate transport. Delivering our services within the home environment or locally within the community helps us to deliver more person-centred care, promotes independence, protects wellbeing, and safely avoids unnecessary hospital admissions. This is especially important for our most rural communities and those who may find it difficult to access our services. We work in partnership with Devon County Council and other organisations to ensure our patients, carers and families experience the best possible integrated health and social care service. Each team is based at either a community hospital or GP surgery and cover a geographical area. However, teams work flexibly to ensure that all areas can be covered. The community nursing team also works with the local hospices to provide palliative care for those patients with terminal illnesses.
